This Situation-Based Learning project was quickly created for a job opportunity.  

 I created a short, virtual, micro-learning video as a piece of instructor-led training in which I would incorporate group discussion and collaboration during instruction for staff.  (The video is no longer available)

After data is collected, this form is used to create a Specific Behavioral Plan to help modify negative behaviors to ensure a more positive experience for that student in the school environment.

Data Collection Form for Behavior Monitoring in Whole Group

I created this detailed Data Collection Form for educators to monitor and collect data from behavior incidents of Tier 3 students who are in a whole group or Inclusion setting.  It focuses on 2 areas of improvements for each student.

Home to School Communication

Along with the usual phone calls, emails, school calendars, homework, and notes in a backpack,  I also created this detailed slip of paper that I attach to work that was completed in a small group or pull out setting so parents are able to quickly see the accommodations their child was given during instructional times.

Google Form Data Collection

It is often appropriate to collect data through colleagues that I am not able to speak with on a daily basis.  To help ensure data is collected, I have created a quick google form that is emailed to my colleague for them to fill out regarding specific goals that are being monitored for each learner.

This is one of the forms I created and used during online learning.
